Ernstige penicilline allergie

vancomycine iv: ofwel intermittent infuus met een oplaaddosis 25 mg/kg gevolgd door 2x 15 mg/kg (start 12u na begin oplaaddosis) ofwel continu infuus met een oplaaddosis 30 mg/kg gevolgd door 30 mg/kg (start onmiddellijk na oplaaddosis). TDM

gentamicine 1x 3 mg/kg iv

Commentaar

“Antibioticabeleid aanpassen op basis van het microbiologisch onderzoek (zie gerichte therapie).  Bij cultuur negatieve endocarditis steeds consult infectieziekten aanvragen.”
